脊柱侧凸与脊髓异常的影像学评价

摘    要:目的 :探讨儿童脊柱侧凸伴随脊髓异常的发病情况及相应的影像学评价。方法 :2 6例脊柱侧凸患者 ,10例行CT检查 ,应用表面遮盖成像 (SSD)三维重建及部分病例采用在二维重建图像基础进行曲面多层面成像 (CMPR ) ,11例行MRI检查 ,5例行CT和MRI两种检查。结果 :2 6例脊柱侧弯 ,婴幼儿型 4例 ,儿童型 13例 ,青年型 9例 ,其中 8例合并脊髓异常 (前两者占 6例 ) ,且全部见于 17例脊柱左凸患者。结论 :年龄较小者及脊柱左侧凸的患者拟行手术治疗时 ,术前应常规进行MRI检查 ,在伴有复杂椎体及附件畸形时 ,结合多层螺旋CT二维及三维重建技术 ,可为临床提供丰富而实用的信息。

Abstract:Objective:To discuss the incidence of spinal cord abnormality in patients with congenital scoliosis and to evaluate the corresponding imaging studies.Methods:Twenty six patients with congenital scoliosis were studied;of them,15 patients had CT examinations and reconstructed with methods of SSD for all patients,and some of them with methods of CMPR;16 patients had MRI examinations and 5 patients had both CT and MRI.Results:Eight patients had spinal cord abnormality in the 26 patients with scoliosis,six of the eight patients were younger children,and all of them had left scoliosis.Conclusion:MRI examination is indicated in younger children with left congenital scoliosis before operation, reconstruction images of multislice spiral CT can also provide valuable information for clinical diagnosis and treatment.
