原发性骶骨肿瘤的外科治疗

１９８５年至今，作者对２４例原发性骶骨肿瘤实施了手术治疗，其中骶骨大部切除７例；扩大范围半骨盆切除３例；肿瘤刮切术１４例。随访１～６年，６例恶性肿瘤死于远隔部位转移，１例脊索瘤软组织复发．本文介绍了骶骨肿瘤的诊断、手术方法及预后等。特别介绍了腹－腹股沟切口及前后联合入路在骶骨手术中的用途。作者认为：腹－腹股沟切口较常规下腹直切口显露盆骶部结构更清楚；前后同时入路显露肿瘤的方法具有出血少、保护重要神经、显露清楚等优点。

Surgical treatment of primary tumors of the sacrum

A review of 24 cases with primary sacrum tumors was presented. Sacrum resection (7 cases), wide hemipelvectomy resection (3 cases), and curettage plus partial resection (14 cases) were the surgical ways to deal with the sacrum tumors in our department from 1985 to 1991. The duration of follow up ranged from 1 to 6 years. 6 patients with malignant tumor died of metastasis, 1 case with chordoma recurred locally in the soft tissue, and the others(17 cases) did well. The diagnosis, surgical management and prognosis were discussed. The authers discribed the abdomino-inguinal incision and abdominosacral approach in detail.
