十二指肠损伤25例临床分析

目的:探讨十二指肠损伤的诊断和外科治疗方法。方法:回顾性分析25例十二指肠损伤患者的临床资料和5例Cogbill改良憩室化手术再简化的手术治疗。结果:25例患者仅5例术前明确诊断,术中确诊15例,术中漏诊、术后诊断5例,误诊率20%,死亡4例,治愈21例,治愈率84%。十二指肠损伤的早期诊断和早期及时手术是非常重要的。结论:十二指肠损伤的病情复杂,目前尚无好的特异性检查,处理困难。十二指肠修补术、十二指肠空肠Roux-en-Y吻合术和Cogbill改良憩室化手术再简化手术是治疗十二指肠损伤的实用方法。

Analysis of Duodenal Injury

Objective: To study the early diagnosis and suitable surgical treatment for duodenal injury.Method: Clinical data of 25 patients with duodenal injury were analyzed retrospectively and 5 patients were successfully with Modificed cogbill's technigues for diverficalization.Result: 25 cases underwent surgical treatment.21 were cured.4 died .Early diagnosis and treatment for duodenal injury were very important.Conclusion: Primary repair and Roux-en-Y duodenojejunostomy and Modificed Cogbill's technigues for diverficalization are practical operation.
