长期低剂量罗红霉素治疗慢性鼻窦炎疗效观察

目的研究长期低剂量口服罗红霉素治疗慢性鼻窦炎的疗效和安全性。方法46例在门诊就诊的慢性鼻窦炎患者接受罗红霉素150mg／d连续口服12周的治疗，分不伴鼻息肉组和伴鼻息肉组，在治疗前和治疗后第12周进行症状和鼻内镜评分，评估疗效和监测副作用。结果44例患者完成试验。用症状视觉模拟量表评分（visualanaloguescale，VAS）评价整体症状和各具体症状。两组患者整体症状、鼻塞、流涕和头痛VAS评分及内镜评分都较治疗前显著改善（P〈0．05），嗅觉VAS评分无明显改善。不伴鼻息肉组整体VAS评分，鼻塞、流涕评分和内镜评分的改善优于鼻息肉组（P〈0．05）。因皮肤过敏和胃肠道反应退出试验各1例。结论长期低剂量应用罗红霉素对慢性鼻窦炎患者有良好疗效，副作用小，值得临床医生尝试.

Clinical observation on the treatment of chronic sinusitis with long - term low dose roxithromycin

Objective To study the effects and safety of treating chronic sinusitis with long - term, low dose roxithromycin. Methods Forty - six out - patients with chronic sinusitis and nasal polyps were administered roxithromycin 150mg/d for 12 weeks. The symptoms and nasal endoscopy score were assessed in baseline and 12th week. Side effects were monitored. Results Forty - four patients accomplished the treatment. Symptoms score and nasal endoscopy score were significantly improved （ P 〈 0.05 ） after treatment excluding olfactory score （P 〉 0. 05 ）. General symptom score, nasal obstruction, rhinorrea score and nasal endoscopy score of CRS without nasal polyps groups were better than CRS with nasal polyps group （ P 〈 0. 05 ）. Two patients quit of the treatment, one for skin allergy and one for gastrointestinal response. Conclusion Long - term low dose roxithromycin has curative and safe effect in treating chronic sinusitis.
