巨大垂体腺瘤的外科治疗

目的探讨巨大垂体腺瘤外科手术治疗的方法及经验。方法对42例巨大垂体腺瘤分别采用神经内镜经蝶窦，显微镜下经额下、额下-翼点等手术入路施行手术治疗。结果42例巨大垂体腺瘤全切13例，近全切24例，大部切除5例，无手术死亡病例。结论选择适当的手术入路是提高巨大垂体腺瘤全切率、减少并发症、降低病残牢及病死率的重要手段。

Surgical treatment of giant pituitary adenomas

Objective To explore the surgical treatment and experience in treating giant pituitary adenomas. Methods The clinical data of 42 cases of giant pituitary adenomas were analyzed retrospectively. The tumors were removed respectively by various operative approaches such as transsphenoidal with neuroendoscopy, transsubfrontal, subfrontal - transpterional ones with microsurgery. The choice of the surgical approach and the points for attention to the operation were introduced in the patients with giant pituitary adenomas. Results Of 42 patients with giant pituitary adenomas, 13 received total removal of the tumors,24 subtotal and 5 greater part. No patients died. Conclusion The appropriate approach,operative opportunity and post - operation radiotherapy can improve prognosis of the patients with giant pituitary adenomas.
