急性肠绞窄缺血术后肠管活力变化及监测的实验研究

摘    要:目的 探讨肠绞窄缺血术后肠管活力的变化情况及建立监测方向。方法 不同情况(缺血时间、缺血肠管长度)的绞窄缺血肠管病例的胃内给予微量的同位素标记物^99m“Tc-DTPA(二乙烯三胺戊乙酸)作功能示踪(口服耐量实验法)并结合组织形态学的方法研究绞窄缺血肠管在恢复血供后的不同时相肠管功能和组织损伤变化的情况。结果 绞窄缺血肠管术后血液对^99m Tc-DTPA的吸收与肠缺血损害的严重程度密切相关,与缺血肠管长度关系不大。腹腔液的观察能很敏感地反映出肠穿孔。结论对绞窄缺血肠管在恢复血供的同时向胃内给予少量^99mnTc-DTPA能够为术后肠管活力的变化情况(包括组织损伤的不同程度和肠穿孔等情况)提供一个敏感而实用的监测指标。此法可望成为监测急性肠缺血术后肠管活力变化的一种新方法。

Abstract:Objective To investigate the changes of bowel vitality and develope a method for monitoring the vitality of strangulated ischemic intestine after repcrfusion.Methods trace isotope maker ~(99m)Tc-DTPA was given by orally(experimental method of tolerance by orally) and method of morphology was combinated to determine the damage degree and function of strangulated ischemic intestine after reperfusion. Results the blood absorption rate of ~(99m)Tc DTPA and the severity degree(not the length of bowel) of strangulated ischemic intestinal damage have a very close relationship in the study. The intestinal perforation could be found sensitively by determine the radioactivity in peritoneal fluid. Conclusions ~(99m)Tc DTPA was used in the experimental method of tolerance by orally is both sensitive and utility fo rmonitoring the viablity(include bowel perforation) of strangulated ischemic intestine after reperfusion.
