Diagnostic Value of Combined Test of Serum Levels of Tumor Markers for Colo-reactal Cancer

Objective To study the diagnostic value of combined test of serum tumor markers for colorectal cancer.Methods Serum CEA,CA19-9,CA72-4 and CYFRA21-1 levels were determined with ECLIA in 101 patients with colonic cancer and 89 patients with rectal cancer both before and after surgery as well as in 50 controls.Results The positive rates of CEA,CYFRA21-1 CA72-4 and CA19-9 in the patients were 60.2%,48.7%,45.7%and 42.8%(mean concentration 29.5±18.6ng/ml,52.4±19.2ng/ml, 16.7±9.4ng/ml and 13.1±10.5ng/ml) respectively,being significantly higher than those in the controls(P<0.01).With combined test of the four markers,if only 1 marker positive was enough to be considered as diagnostic,the sensitivity would be 84.5%but the specificity was only 73.8%.If all the markers must be positive,then the specificity would be 100%(sensitivity as low as 29.9%).The levels of the markers were significantly higher in patients of Dukes B,C stages than those in patients of Dukes A stage (P<0.01).After surgery,all the levels dropped significantly(vs before operation,P<0.01).Conclusion Combined test of serum tumor markers levels would increase the diagnostic sensitivity and specificity greatly.
