Comparative study of laparoscopic-assisted and mini-incision gallbladder-preserving cholelithotomy

Objective:To investigate the feasibility and safety of laparoscopic-assisted and mini-incision gallbladder-preserving cholelithotomy in treatment of cholecystolithiasis. Methods:The clinical data of 320 patients who suffered from cholecystolithiasis and underwent gallbladder-preserving cholelithotomy were retrospectively analyzed.Before operation,the visualization of gallbladder was favorable,the cholecystic wall was less than 4mm according to B-ultrasound,and the shrinkage of cholecyst was 30%.Patients were randomly and equally divided into observation group and control group.Patients in observation group underwent laparoscopic-assisted choledochoscopic gallbladder-preserving cholelithotomy,and control group underwent mini-incision choledochoscopic gallbladder-preserving cholelithotomy.The incision length,operative time and intraoperative blood loss in 2 groups were analyzed. Results:The gallbladder-preserving cholelithotomy in 2 groups was all successfully performed,incision length,operative time and intraoperative blood loss in observation group were better than control group(P<0.05). Conclusions:Laparoscopic-assisted gallbladder-preserving cholelithotomy is better than mini-incision approach in incision length,operative time,intraoperative hemorrhage and conversion to cholecystectomy.
