肺结节病合并胸腔积液14例诊治分析

目的对肺结节病合并胸腔积液的诊治进行分析。方法对我院1991~2009年间,肺结节病合并胸腔积液的14例患者进行分析。结果 8例为Ⅱ期,6例为Ⅰ期。13例口服醋酸泼尼松片（0.5 mg/kg）逐渐减量治疗。3例曾联合抗结核治疗。口服激素1个月后,4例胸腔积液消失,4例明显减少,3个月后14例患者胸腔积液均消失,1例留有胸膜肥厚。结论肺结节病出现胸腔积液的机制不明,激素治疗剂量与无胸腔积液患者用量相同,逐渐减量。

Clinical analysis of pleural effusion in 14 cases with pulmonary sarcoidosis

Objective To discuss the clinical features of pulmonary scarcoidosis with pleural effusion,and analysis the diagnosis and treatment.Methods Analyzed retrospectively the 14 cases of pulmonary scarcoidosis with pleural effusion from 1991 to 2009 in our hospital.Resaults 8 cases were Stage Ⅱ pulmonary sarcoidosis,6 cases were Stage Ⅰ.13 cases initial dose of glucocorticoid,prednisone was 0.5 mg/kg per day,and decreased every other 2~3 months.After glucocorticoid treatment one month,4 cases had no more pleural effusion,4 cases’ pleural effusion obviously decreased.The pleural effusion of 14 cases disappeared after 3 month.One case had pachynsis pleurae.Conclusions The formation mechanism of pulmonary scarcoidosis with pleural effusion needs to be investigated deeply.The dose of glucocorticoid treatment about pulmonary scarcoidosis with pleural effusion is same to with no pleural effusion.After pleural effusion disappears,glucocorticoid treatment needs to be continued,and decreased gradually.Anti-tuberculosis treatment is aim to prevent the misdiagnosis of tuberculosis.
