急性胰腺炎高敏C反应蛋白水平与CT分级的对照研究

目的探讨高敏C反应蛋白（hs-CRP）水平与CT分级在急性胰腺炎（AP）的诊断、严重程度及预后评估中的价值。方法动态观察48例轻症急性胰腺炎（MAP）患者和51例重症急性胰腺炎（SAP）患者的hs-CRP水平,并与其相应的Balthazar CT分级相比较分析。结果48例MAP患者hs-CRP水平在正常值的10～20倍之间,其Balthazar CT分级为A级12例,B级28例,C级8例;51例SAP患者hs-CRP水平均高于正常值30倍以上,其CT分级为D级29例,E级22例。Spearman相关性分析显示,hs-CRP水平与CT分级成正相关（r=0.864,P=0.000）。轻症与重症组之间hs-CRP水平比较,差异具有统计学意义（P〈0.05）。结论 Balthazar CT分级可以准确反映SAP的病理变化,但对MAP的诊断有一定困难。动态监测hs-CRP水平可作为AP早期诊断、严重程度及预后判断的一个独立的评估指标。

Control study of acute pancreatitis in high-sensitivity C-reactive protein levels and CT grading

Objective To explore the value of high-sensitivity C-reactive protein（hs-CRP） levels and CT grading in the diagnosis and prognosis of acute pancreatitis（AP）.Methods The 48 cases of mild acute pancreatitis（MAP） patients and 51 cases of severe acute pancreatitis（SAP） in patients with hs-CRP levels were dynamic observed and its corresponding Balthazar CT classifications were comparative analyzed.Results The hs-CRP levels in 48 cases of patients with MAP were 10 to 20 times higher than the normal,the Balthazar CT grading for 12 cases of Class A,28 cases of Grade B,8 cases of Class C;hs-CRP of 51 patients with SAP were 30 times higher than normal,the CT grading for 29 cases of Grade D,22 cases in E-Class.Spearman correlation analysis showed that hs-CRP levels and CT grading were positively correlated（r=0.864,P=0.000）.Hs-CRP levels between mild and severe group,the difference was statistically significant（P0.05）.Conclusion Balthazar CT classification can accurately reflect the pathological changes of SAP,but the diagnosis of the MAP are certain difficulties.Dynamic monitoring of hs-CRP levels can be used as an independent evaluation indicator for AP early diagnosis,severity and prognosis.
