Wiltse肌间隙入路治疗57例胸腰段无神经损伤症状的单节段椎体骨折

目的探讨经Wiltse肌间隙入路内固定治疗胸腰段单椎体压缩性骨折的手术方法及临床疗效。方法自2007年10月—2010年6月我院对57例无神经损伤表现的胸腰段单节段椎体骨折患者行Wiltse肌间隙入路后路钉棒系统复位固定。结果手术时间75～115min,平均（87±23.8）min,出血量85～175ml,平均（135±51.7）ml;全部病例均获得12～26个月随访,所有病例未发现内固定失效及腰背疼痛症状。结论经Wiltse肌间隙入路治疗无神经损伤症状的胸腰段单节段骨折手术方式具有创伤小,操作简便,术后恢复快,患者满意度高的优点。

The treatment for the thoracolumbar simple vertebral fracture without neurologic deficits through Wiltse paraspinal approach

Objective To explore the methods and results of the treatment for the thoracolumbar simple vertebral fracture without neurologic deficits through the Wiltse paraspinal approach. Methods A total of 57 patients with the thoracolumbar simple vertebral fracture underwent posterior open reduction and internal fixation from October 2007 to June 2010 in our hospital. Results Median operating time was 75-115min,（87±23.8）min on average and blood loss was 85-175ml,（135±51.7）ml on average.All the patients were followed up for 18-26 months.There was no instance of instrumentation failure and no patient had persistent postoperative back pain. Conclusion Wiltse paraspine approach in treatment of thoracolumbar simple vertebral fracture has the advantages of minimally invasive,simple,rapid postoperative recovery and high patient satisfaction.
