血液恶性肿瘤参数的变化和意义

目的探讨血液肿瘤患者红细胞参数的变化及其临床意义。方法正常对照组60例;血液肿瘤71例,其中多发性骨髓瘤（MM）组12例,非霍奇金淋巴瘤（NHL）组18例,霍奇金病（HD）组4例,慢性粒细胞性白血病（CML）组14例,急性白血病（AL）组23例。采用全自动血细胞分析仪检测研究对象外周血的红细胞参数。结果五组血液肿瘤外周血红细胞数（RBC）、血红蛋白量（HB）、红细胞比容（HCT）均明显下降,而红细胞体积分布宽度（RDW）均明显升高,尤其以CML、AL组升高明显;红细胞平均体积（CMV）在NHL组明显下降,而CML组升高,MM、HD、AL组无明显差异;红细胞平均血红蛋白量（MCH）在MM、AL组升高,在NHL、HD组下降,CML组无变化;红细胞平均血红蛋白浓度（MCHC）在MM、NHL、HD、CML组下降,而AL组升高。结论血液肿瘤对于红系造血功能的影响,有红细胞数量和质的双重变化,观察红细胞参数的变化,对于血液肿瘤的诊断、治疗、疗效判断都具有重要的临床意义。

The variation and meaning of parameters of red blood cells in blood system malignant tumors

Objective To discuss the variation and clinical meaning of parameters of red blood cells in blood system tumors.Methods There were 60 examples in control group,and 71 examples of blood tumors,including 12 multiple myeloma（MM） examples,18 non-hodgkin＇s lymphoma（NHL） examples,4 Hodgkin＇s disease（HD） examples,14 chronic myelogenous leukemia（CML） examples and 23 acute leukocythemia（AL） examples.We used full-automatic blood cell analyzer to detect the parameters of red blood cells.Results The number of red blood cells,HB and HCT of blood tumors group decreased,but RDW increased especially in CML and AL group.The CMV in NHL group decreased obviously,but in CML group increased which no obvious difference in MM,HD and AL group.The MCH increased in MM,AL group,but decreased in NHL and HD group.The MCHC decreased in MM,NHL,HD and CML group,but in AL group increased.Conclusion The effects of blood system tumors on hematopoietic function has two sides,one on quantity of RBC,the other one on the quality of RBC.Observing the variation of parameters of red blood cells is meaningful in diagnosis,treatment and judging the effects in clinical.
