曲美他嗪治疗冠心病心力衰竭的临床疗效及对血浆脑钠肽的影响

目的观察曲美他嗪治疗冠心病心力衰竭的临床疗效、安全性及对血浆脑钠肽的影响。方法108例慢性心力衰竭患者随机分为两组,对照组予以血管紧张素转换酶抑制剂、β受体阻滞剂、正性肌力药物、利尿剂及血管扩张剂等药物治疗,治疗组在此基础上加用曲美他嗪20 mg,3次/d口服,治疗6个月后对比观察两组临床症状、左室射血分数、左室舒张末期内径、6 min步行距离、血浆脑钠肽的变化情况。结果治疗组各项指标均明显改善,患者生活质量明显提高。结论在常规抗心力衰竭治疗基础上加用曲美他嗪治疗冠心病心力衰竭安全有效,且能改善冠心病心力衰竭患者的生活质量,具有重要的临床意义。

Effect and Influence of Trimetazidine for Plasma Levels of Brain Natriuretic Peptides and Heart Failure in Patients with Coronary Heart Diease

Objective To observe the curative effect and influence of trimetazidine for the plasma levels of brain natriuretic peptides（BNP） and heart failure of patients with coronary heart diease.Methods 108 patients with coronary heart diease combined with heart failure were divided randomly into two groups,one was control group treated with ACE inhibitor,β-Blockers,diuretic,digitoxin and vasodilator,another was treatment group treated with trimetazidine on the basis of above regular treatment.All the patients were followed up for 6 months and observed the changes of the clinical symptoms,heart function,left ventricular ejection fraction（LVEF）,left ventricular end diastolic diameter（LVEDd）,6 minutes walking distance and change of plasma levels of BNP.Results After trimetazidine treatment,each index of the treatment group was obviously improved and the LVEF was higher than control group（P〈0.01）,LVEDd was much more reduced than that of compared with control group（P〈0.01）,BNP plasma levels both decreased significantly（P〈0.01）,BNP plasma levels of treatment group more decreased significantly（P〈0.01）.Conclusion Trimetazidine can be the effective drug used in clinic for coronary heart diease combined with heart failure.
