大肠癌门静脉插管化疗预防肝转移

目的 :探讨大肠癌根治术门静脉插管 5 -Fu化疗预防肝转移的疗效。方法 :87例DukesB、C期大肠癌患者 ,随机双盲分组 :根治性切除加门静脉化疗 45例 (治疗组 )及单纯根治性切除 42例 (对照组 )。治疗组于根治术当日即以 5 -Fu 1g持续 2 4小时门静脉滴注化疗 7天 ;对照组住院期间不作辅助化疗。结果 :治疗组肝转移发生率明显低于对照组 ,DukesC期术后 3 ,5年生存率也显著高于对照组 ,DukesB ,期无明显差异。结论 :大肠癌根治术门静脉插管预防性化疗 ,可有效降低肝转移的发生率 ,提高DukesC期患者 3、5年生存率。

Preventing Hepatic Metastasis by Port Vein Infussion on the Patients with Large Bowel Cancer

Objective:To investigate the effect of the chemotherapy by port vein infussion (PVI) to prevent hepatic metastasis on the patients with large bowel cancer.Methods:After radical resection ,87 patients with large bowel cancer were randomly divided into 2 groups, the therapy group received 5-Fu (1.0g/d, continued 7d) chemotherapy by PVI the poeration day. The control group did not receive any chemotherapy during the time in the hospital.Results:The rate of liver metastasis in the therapy group was lower than in the control group and the rate of 3 and 5-year-survival had no difference in stage Dukes B, but it was higher in the chemotherapy of PVI group than the control group in stage Dukes C.Conclusions:5-Fu chemotherapy by PVI after radical resection in the patients with large bowel cancer may decrease the liver metastasis rate and raise the 3 and 5-year-survival rate in stage Dukes C.
