帕罗西汀联合血府逐瘀汤治疗老年冠心病抑郁状态的疗效观察

目的探讨血府逐瘀汤联合帕罗西汀治疗老年冠心病抑郁状态的临床疗效。方法选择234例冠心病伴发抑郁症的老年患者，平均随机分为对照组、治疗组及联合组。对照组接受常规冠心病治疗，治疗组在对照组基础上接受口服帕罗西汀治疗；联合组则在治疗组基础上接受口服血府逐瘀汤治疗。观察并比较3组患者冠心病临床疗效；治疗组与联合组抑郁症的临床疗效以及2组患者治疗前后汉密尔顿抑郁量表（HAMD）评分。结果与对照组比较，治疗组及联合组冠心病临床总有效率显著提高（P均〈0．05），其中以联合组提高最为明显（P〈0．05）；与治疗前相比，治疗组及联合组HAMD评分均显著降低（P均〈0．05），以联合组降低最为明显（P〈0．05）；联合组抑郁症临床总有效率显著高于治疗组（P〈0．05）。蛄论血府逐瘀汤联合帕罗西汀适合老年冠心病合并抑郁症的治疗，优于单用帕罗西汀。

Observation of effects of combination therapy of paroxetine and Xuefuzhuyu decoction on coronary heart disease complicating with depression in elder patients

Objective It is to explore the combination therapy of paroxetine and Xuefuzhuyu decoction on coronary heart disease （CHD） complicating with depression in elder patients. Methods 234 elderly patients with CHD complicating with de- pression were selected and divided evenly and randomly into control group, treatment group and combination group. Control group received regular treatment of CHD ; treatment group received treatment of paroxetine based on treatment in control group ; combination group received treatment of Xuefuzhuyu decoction based on treatment in treatment group. Clinical efficacy on CHD in 3 groups was observed and compared; HAMD score in treatment group and combination group was compared before and after treatments. Results Compared with control group, total clinical efficacy rates of CHD were obviously increased in combination group and treatment group（ P 〈 0.05 ） , and the rate in combination group was the highest （P 〈0.05 ）. HAMD score in treat- ment and combination groups decreased significantly after treatments, but more significantly in combination group （ P 〈 0.05 ） ; total clinical efficacy of depression increased more significantly in combination group than treatment group. Conclusion Combi- nation therapy of paroxetine and Xuefuzhuyu decoction is suitable in the treatment of CHD complicating with depression, and its effect was better than that of paroxetine administration alone.
