中性粒细胞与淋巴细胞比值对评估晚期胃癌患者预后的临床价值

目的：探讨中性粒细胞与淋巴细胞比值（neutrophil to lymphocyte ratio，NLR）对晚期胃癌患者预后的预测价值。方法选取72例接受胃癌根治术的晚期胃癌患者作为研究对象，根据血细胞分类计数计算NLR ，将53例NLR＜3的患者列为低NLR组；19例NLR≥3的患者列为高NLR组，分析两组患者的临床病理特征和预后。结果高NLR组中浸润深度为T3、T4期患者的比例占89.47%，淋巴结转移中N1、N2、N3的比例占63.16%，远处转移中M1的比例为15.79%；低NLR组中浸润深度为T3、T4期患者的比例占77.36%，淋巴结转移中N1、N2、N3的比例占50.94%，远处转移中M1的比例为7.55%。由此，高NLR组中出现浸润深度、淋巴结转移、远处转移的例数明显高于低NLR组，差异有统计学意义（P＜0.05）；低NLR组和高NLR组的5年生存率（58.49% vs 42.11%）及无病生存率（73.58% vs 57.89%）差异显著，均具有统计学意义（P＜0.05）。结论高NLR可作为晚期胃癌患者生存率、无病生存率的独立危险因素，并对评估患者的预后具有重要的预测作用。

Neutrophil-lymphocyte ratio of the prognosis of patients with advanced gastric cancer predictive value

Objective To investigate the neutrophil to lymphocyte ratio (NLR) predictive value in patients with advanced gastric cancer prognosis. Method Patients with advanced gastric cancer between 72 routine gastrectomy as the research object, and calculates the NLR in accordance with peripheral blood cell count, and 33 cases of NLR<3 patients classified as low NLR group, 19 patients classified as high NLR≥3 NLR group, clinical features and progno-sis of patients two groups of patients were analyzed, analysis of neutrophil-lymphocyte ratio prognostic value in pa-tients with advanced gastric cancer. Result The depth of invasion NLR group T3, T4 ratio of 89.47%, lymph node metastasis N1, N2, N3 ratio of 63.16%, distant metastasis M1 ratio was 15.79%, the low NLR group, depth of invasion, T3, T4 ratio of 77.36%, lymph node transfer proportion of N1, N2, N3 of 50.94%, distant metastasis M1 ratio of 7.55%, higher NLR group depth of invasion, lymph node metastasis, distant metastasis was significantly higher than the low NLR group, the difference was statistically significant(P<0.05). Low NLR group of 5-year survival rate was signifi-cantly higher than 58.49% 42.11% higher NLR group, the difference was statistically significant(P<0.05). Low NLR group of disease-free survival rate 73.58%(39/53) was significantly higher than the high NLR group 57.89%(11/19), the difference was statistically significant(P<0.05). Conclusion NLR as advanced gastric cancer patients survival, disease-free survival as an independent risk factor, and its value for the prognosis of patients with advanced gastric cancer has an important predictive value.
