氯诺昔康在颈部手术中超前镇痛的疗效观察

目的观察氯诺昔康在颈部手术中超前镇痛的有效性和安全性。方法 60例择期行颈部手术患者随机分为超前镇痛组（A组）和术后镇痛组（B组）,每组各30例,均采取颈丛阻滞麻醉。A组在术前30min静注16mg氯诺昔康,手术后静注生理盐水10ml;B组在手术前30min静注生理盐水10ml,手术后静注16mg氯诺昔康。两组术后均未使用自控镇痛,采用视觉模拟评分法（VAS）单盲评估患者术后1h、2h、4h、8h、12h和24h的疼痛感觉程度,并观察与氯诺昔康有关的不良反应。结果 A组术后4h内VAS评分显著低于B组（p〈0.05）,两组4h后VAS评分无显著性差异（p〉0.05）。结论氯诺昔康超前镇痛可有效缓解颈部手术术后疼痛,且不良反应少见,但尚不能完全解决术后疼痛。

Efficacy of Lornoxicam for Preemptive Analgesia after Neck Surgery

Objective To evaluate effects of lornoxicam for preemptive analgesia in patients undergoing neck surgery.Methods Sixty patients undergoing neck surgery were randomly divided into two groups：A and B.A standardized nerve block anesthesia was used.Patients in group A（n=30） received lornoxicam 16mg 30 minutes before induction;patients in group B（n=30） received the same doses lornoxicam（16mg） after skin closure.All patients were not used of postoperative analgesia.The effectiveness was assessed postoperatively using the visual analogue scale（VAS） at 1、2、4、8、12、24 hours after surgery,and side effects were also recorded.Results At each observed point during 4h after surgery,the VAS of patients in group A were significantly lower than in group B（p 〈0.05）.The VAS were same in the two groups at the time point after 4h（p0.05）.Conclusion Preemptive analgesia with lornoxicam 16mg could provide satisfactory pain relief with no obvious side effects after neck surgery.But it can not solve the postoperative pain completely.
