三叉神经鞘瘤的外科治疗

目的 :总结三叉神经鞘瘤的诊断及治疗经验。方法 :本院收治的 15例患者在行CT、X线和MRI检查后确诊。手术 14例 ,γ -刀治疗 1例。本组手术入路有经额颞入路和后枕下入路。结果 :除 1例位后颅凹的肿瘤因与脑干粘连紧密 ,在行次全切除后遗留有一侧面瘫及一侧肢体轻偏瘫外 ,其余肿瘤均予全切除 ,且术后未增加患者的神经功能损害。结论 :三叉神经鞘瘤的诊断主要依赖X线、CT和MRI。治疗方法主要是外科手术治疗和立体定向放射神经外科治疗。

Surgical treatment of trigeminal neurinomas

Objective:To study the diagnosis and treatments for the patients with trigeminal neurinomas.Methods:Fifteen patients with trigeminal neurinomas were diagnosed by the means of CT,X-ray and MRI.Fourteen patients were operated,and 1 patient was treated by γ-Knife.The operating approachs included subtemporal and suboccipital craniotomy in this group.Results:One patient received a subtotal resection of the tumour which adhered tightly to the brainstem,and gained one side facial and extremity paresis after operation.All of other patients received total resection of the tumours had not gained new signs and symptoms of neurological dysfunction.Conclusion:The diagnosis of the trigeminal neurinomas mainly depends on X-ray?CT and MRI.The main therapeutic methods are neurosurgery and the stereotactic radiosurgery.
