先天性心脏病合并肺动脉高压的内皮素改变及其意义

应用放射免疫分析法测定３８例先天性心脏病患者血浆内皮素浓度（肺动脉高压组１５例，非肺动脉高压组２３例）。肺动脉高压组内皮素浓度明显高于非肺动脉高压组，肺动脉血浆内皮素浓度明显高于右心室和股动脉；内皮素浓度与肺动脉压力及肺血管阻力呈明显正相关。提示血浆内皮素增高在肺高压形成中具有重要的病理生理意义。

THE CHANGES OF PLASMA ENDOTHELIN CONCENTRATION AND ITS CLINICAL SIGNIFICANCE IN PULMONARY HYPERTENSION ASSOCIATED WITH CONGENITAL HEART DEFECTS

Plasma endothelin(ET) concentration was measured by means of radioimmunoassay in 38 patients with congeital heart defects among whom 15 patients with pulmonary hypertension(PH) and 23 patients without pulmonary hypertension(non-PH). Blood samples were obtained separately from the femoral vein, right atrium, right ventricle, main pulmonary artery and the femoral artery during catheterization. Plasma ET concentration in the PH group was significantly higher than that in the non-PH group at all four sampling sites. In the PH group plasma ET concentration in the pulmonary artery showed higher than that of the right ventricle and the femoral artery, which was significantly in positive correlation with pulmonary artery pressure. It is considered that the elevation of ET plays an important physiological role in the formation of pulmonary hypertension.
