64层螺旋CTA在老年缺血性脑血管疾病中的应用

摘    要:目的通过与数字剪影血管造影术(DSA)的比较,评价64层螺旋CT血管造影(CTA)诊断老年缺血性脑血管疾病的临床应用价值。方法对68例60岁及以上拟诊为缺血性脑血管病的患者进行64层螺旋CTA检查.其中28例在CTA检查1周内行全脑血管DSA检查,对比分析DSA及CTA的检查结果。结果以DSA为参考标准,CTA检出老年脑血管中度狭窄的敏感度和特异度分别为97.6%和99.2%,重度狭窄为87.5%和99.3%,闭塞分别为92-3%,99.8%。结论64层螺旋CTA能清晰显示颅内血管狭窄,是老年缺血性脑血管病诊断的可靠方法。

Abstract:Objective To evaluate the clinical value of 64-slice spiral CT angiography (CTA) in elderly patients with ischemic cerebrovaseular disease through comparison with digital subtraction angiography(DSA). Methods Sixty-eight patients over the age of 60 who were prediagnosed as ischemic cerebrovascular disease underwent 64-slice spiral CTA, and 28 of them received DSA within 1 week of CTA. Results of CTA were analyzed and compared with the reference standard DSA. Results The sensitivity and specificity of CTA were, respectively, 97.6% and 99.2% in moderate stenosis, 87.5% and 99.3% in severe stenosis, and 92.3% and 99.8% in occlusion. Conclusion The 64-slice spiral CTA as a reliable method can clearly demonstrate internal carotid artery stenosis in elderly patients with isehemic cerebrovascular disease.
