三维调强适形放疗联合热疗治疗局部晚期非小细胞肺癌的疗效及不良反应的研究

摘    要:目的:评价三维调强适形放疗联合深部热疗治疗局部晚期非小细胞肺癌(NSCLC)的临床疗效和不良反应。方法:回顾性分析2010年4月~2011年9月采用三维调强适形放疗单用或联合深部热疗治疗局部晚期NSCLC患者76例,其中,单纯行放疗组(A组)36例,放疗联合热疗组(B组)40例。完成治疗后0~3个月通过CT对近期疗效及肺损伤进行评价。结果:两组有效率分别为A组52.8%,B组77.5%。A组与B组有效率比较,差异有统计学意义(P<0.05);A组与B组的肺损伤的发生率和级别比较,差异有统计学意义(P<0.05)。结论:三维调强适形放疗联合热疗治疗局部晚期NSCLC的近期疗效优于单纯行三维调强适形放疗,且放疗联合热疗可有效降低放射性肺损伤的发生率和发生程度。

Abstract:Objective To evaluate the clinical efficacy and toxicity of three-dimensional intensity modulated radiotherapy therapy combined with deep hyperthermia treatment of locally advanced non-small cell lung cancer(NSCLC).Methods A retrospective analysis of three-dimensional intensity modulated radiotherapy therapy alone or in combination with deep hyperthermia treatment of locally advanced NSCLC patients in our hospital from April 2010 to September 2011 76 cases,of which,a simple radiotherapy group(A) 36 cases,radiotherapy combined with hyperthermia group(group B) 40 cases.CT evaluation of recent efficacy and lung injury were conducted after completion of treatment in 3 months.Results The efficient ratio in group A was 52.8% while in group B was 77.5%.There was significant different between group A and B with the efficient ratio(P<0.05).At the same time,there was significant difference between group A and B with the incidence of lung injury and the level of comparison(P<0.05).Conclusion The effectiveness of three-dimensional intensity modulated radiotherapy therapy combined with hyperthermia treatment of locally advanced NSCLC was better than a simple line of three-dimensional intensity modulated radiotherapy therapy.In a word,radiotherapy combined with hyperthermia effectively reduced the incidence of radiation-induced lung injury and the occurrence degree.
