云南省昆明地区高危人群HIV-1二重感染的研究

摘    要:目的了解云南省昆明地区高危人群艾滋病病毒I型(HIV-1)二重感染的发生情况与特征。方法筛选昆明地区高危人群出现HIV-1二重感染的病例。通过不同的PCR/测序引物得到不同的测序结果,或直接测序时碱基模糊不清而确定的可疑二重感染病例,然后采用TA克隆的方法对单个PCR克隆进行测序,找到血浆标本中的HIV-1准种,并分析其系统发生和重组结构。结果昆明地区的高危人群中,HIV-1二重感染的发生率为7.7%(2/26)。2名具有高危异性性接触史的HIV-1感染者,分别为CRF01_AE和CRF08-BC或CRF07-BC的二重感染。结论昆明地区高危人群HIV-1的二重感染较为常见,这为快速产生新的重组病毒株提供了基础。该研究提示,该地区高危人群经常暴露于新的病毒株,将加速基因型内重组的发生。

HIV-1 dual infection among high-risk populations in Kunming of Yunan Province

Abstract:Objective To investigate the occurrence and the characteristics of HIV-1 dual infection among high- risk populations in Kunming of Yunnan Province. Methods High-risk populations were screened for HIV-1 dual infection in Kunming. Suspected cases of dual infection were identified by findings of discordant sequence obtained with different PCR/sequencing primers or by ambiguous readings in direct sequencing. HIV-1 quasispecies in plasma were then characterized by clonal sequence analysis of independent PCR clones generated by TA cloning method, and their phylogeny and recombinant structure were examined. Results Dual infection was detected at a frequency of 7.7M (2 of 26) among high-risk populations in Kunming. CRF01_AE and either CRF08 BC or CRF07_BC were i- dentified in two individuals with high-risk behaviors. Conclusion Dual infections are relatively common and provide the basis for rapid generation of new recombinant HIV-1 strains in Kunming. This implies that HIV-1 intergenotype recombination is accelerated by frequent exposures to new viruses in this region.
