179例HIV／AIDS病人肝脏损害的临床研究

目的探讨艾滋病病毒（HIV）感染者／艾滋病（AIDS）病人的肝脏功能损害情况及其影响因素。方法回顾分析179例HIV／AIDS病人临床资料，根据肝脏功能损害程度，分为肝功能正常组、肝功能轻度损害组、中度损害组、重度损害组，比较各组间相关情况。结果肝功能损害组使用肝毒性药物、合并HBV和／或HCV感染的比例均高于肝功能正常组，两者比较差异有显著的统计学意义（P值分别为0．039、0．030、0．003）。中重度肝功能损害组使用肝毒性药物的比例高于轻度损害组，两者比较差异有显著的统计学意义（P=0．037）。结论HIv／AIDS病人肝脏损害发生率高，而男性、使用肝损害药物、合并HBV和／或HCV是HIV／AIDS病人发生肝脏损害的危险因素。

Clinical study of factors related to liver damage in people living with HIV/AIDS

Objective To investigate liver damage and related risk factors in people living with HIV/AIDS. Methods Clinical data of 179 hospitalized HIV/AIDS cases were retrospectively studied, The liver damage was di- vided into 4 groups according to its grade： group with normal liver functions, group with mild liver dysfunctions （group A）, group with moderate liver dysfunctions （group B）, group with severe liver dysfunctions （group C）. The factors which might be related to liver damage were compared among the four groups. Results Among males the proportion of using hepato-toxic drugs and infection with HBV/HCV in the 3 groups with different degrees of liver dysfunctions was higher than in the group with normal liver functions （P= 0. 039, 0. 030, 0. 003, respective- ly）. The proportion of using hepato-toxic drugs was significantly higher in the group B and C than in the group A （P~0. 037）. Conclusion In males HBV/HCV co-infection and exposure to hepato-toxic drugs may be the most im- portant risk factors related to liver damage in people living with HIV/AIDS in China, and HAART would aggravate the damage of the liver.
