实时三维超声心动图在二尖瓣病变诊断中的应用价值

目的探讨实时三维超声心动图（RT-3D）在二尖瓣病变诊断中的应用价值。方法 24例成功行二尖瓣置换术的患者术前及术后1个月均行二维超声（2DE）及RT-3D检查,术前观察二尖瓣病变部位及范围,并与手术所见对比;术后观察人工瓣结构及功能。结果风湿性心脏病二尖瓣狭窄15例,2DE及RT-3D均显示二尖瓣严重狭窄,钙化,RT-3D还显示1例合并二尖瓣腱索钙化,与手术所见一致。二尖瓣脱垂9例,2DE及RT-3D对病变区域的定位与手术所见相符,且RT-3D观察病变瓣膜的形态与手术视野一致。24例患者二尖瓣置换术后,2DE无法显示人工瓣形态、运动全貌,而RT-3D显示人工瓣表面回声,启闭良好。结论 RT-3D在二尖瓣病变的诊断中可获得比2DE更丰富的信息,是2DE的有益补充。

Application of real-time three-dimensional echocardiography in the diagnosis of mitral valvular diseases

Objective To investigate the value of real-time three-dimensional echocardiography in the diagnosis of mitral valvular diseases.Methods 24 patients with mitral valvular diseases were underwent 2DE and RT-3D examinations before and after mitral valve replacement surgery.Observe the positions and ranges of mitral valve lesions before operations,and contrast the results with the views of operations.Observe the structure and function of the prosthetic valves after erations.Results 15 cases were rheumatic heart disease mitral stenosis,both 2DE and RT-3D diagnosed they were severe stricture,calcified.RT-3D also diagnosed 1 case had tendinous cord calcification.9 cases were mitral valve prolapse,the results of 2DE and RT-3D were the same as the views of operations,and RT-3D could demonstrate the structure more clearly.2DE could not show the shapes and movements of the prosthetic valves of 24 patients,but RT-3D showed the prosthetic valves clearly.Conclusion RT-3D can provide more information than 2DE in the diagnosis of mitral valvular diseases and offer a useful complement to 2DE.
