LEEP在子宫颈病变诊断与治疗中的临床应用

目的:探讨高频电波刀宫颈电圈环切术(LEEP)对宫颈病变诊治的指征、疗效与复发因素。方法:采用阴道镜辅助下的宫颈环型电切术,对188例宫颈疾病患者进行诊治,并进行12~18个月的随访。结果:慢性宫颈炎97例,治愈率100%;CIN91例,治愈87例,复发4例,总治愈率97.87%;复发率2.13%。手术并发症主要为出血,出血率42.02%(79/188)。LEEP切除组织无CIN41例。结论:LEEP适用于宫颈CINⅡ~Ⅲ的锥切术和宫颈CINⅠ、ASCUS活检术,同时也适用于严重的慢性宫颈炎的治疗;它安全、省时、简单、有效、并发症少,可提供完整的病理标本;但需长期随访,尤其是术后两年内。

Application of loop electrosurgical excision procedure in the diagnosis and treatment of cervical diseases

Objective:To determine the indication,effectiveness of loop electrosurgical excision procedure(LEEP)and factors of recurrence.Methods:LEEP under colposcopic guidance was performed in an outpatient of colposcopic clinic.188 patients of cervical diseases were treaded and followed up for 12-18 months in our department.Results:LEEP was successful in treating 97 patients of chronic cervicitis and 87 patients of CIN,the overall cure rate was 100% and 97.87% respectively.Recurrence CIN occurred in 4 patients(2.13%).Mainly complication is bleeding,it occurred in 79 patients(42.02%);LEEP did not detected CIN in 41 patients.Conclusions:LEEP is indicated to conization of cervix of CIN2-3 and cervical biopsy of CIN1 and ASCUS,and is indicated serious chronic cervicitis.It can offer intact sample for pathological diagnosis and is safe,time-saving,simple,effective,lower complication.But the patients need to be followed for a long time,especially in the first two years.
