胰腺导管腺癌与非功能胰岛细胞瘤的临床对比分析

目的：分析胰腺导管腺癌（ PDAC）与非功能胰岛细胞瘤（ NICT）的临床特点。方法回顾性总结我院收治的15例PDAC与21例NICT的临床症状、CT表现，并对比分析肿瘤学特征与CT影像学关系。结果 PDAC CT平扫可显示肝内、外胆管扩张，胆囊胀大，胰管扩张（正常直径﹤3 mm），胰头部占位性病变，同时又可以观察有无肝转移和淋巴结转移。胰腺区动态薄层增强扫描可获得更佳效果，且不受肠胀气影响。对于瘤体较小的NICT者，CT平扫难以发现，增强扫描动脉期明显强化，静脉期稍微减弱。结论结合临床表现、组织学特征结合CT影像检查有助于PDAC与NICT的鉴别诊断。

Clinical comparative analysis of pancreatic ductal adenocarcinoma and nonfunctional islet cell tumor

Objective To analyze the clinical features of pancreatic ductal adenocarcinoma （ PDAC）and nonfunctional islet cell tumor（ NICT）. Methods The clinical symptoms and CT presentation of 15 PDAC cases and 21 NICT cases were studied retrospectively. Relationship between oncological fea-tures and CT imaging were compared and analyzed. Results For PDAC,plain CT scanning could reveal intrahepatic and extrahepatic expansion of the bile duct,swollen gallbladder,pancreatic duct dilation（ di-ameter﹤3mm）and occupancy lesions of the pancreatic head. At the same time,liver and lymph node me-tastasis could also be observed. Thin-layer scanning with dynamic enhancement could achieve a better effect and it′s not affected by the intestinal tympanites. Most small NICT tumors were difficult to be detec-ted by the plain scanning,but they could be markedly enhanced in the arterial phase and slightly enhanced in the portal venous phase. Conclusion Clinical symptom,pathologic features and CT scanning are help-ful to differentiate PDAC and NICT.
