免打结缝合线在腹腔镜胆总管探查一期缝合术中的临床应用

目的 探讨免打结缝合线（QUILL线）在腹腔镜胆总管探查一期缝合中应用效果.方法 80例腹腔镜胆总管探查术中,38例免打结缝合线一期缝合胆总管（观察组）,42例T管引流（对照组）,比较两组患者术中及术后情况.结果 对照组手术缝合时间、手术时间、住院时间明显长于观察组,差异有统计学意义（P＜0.05）,在术后肠功能恢复时间、腹腔引流时间、胆漏发生率上两者差异无统计学意义（P＞0.05）.结论 免打结缝合线在腹腔镜胆总管探查一期缝合中应用可明显缩短缝合时间,具有操作简便、缩短住院时间等特点,且不增加胆漏的发生率.

Clinical application of knot-free suture for primary suture of laparoscopic common bile duct exploration

Objective To explore the effects of knot-free suture （QUILL suture）for primary suture of laparoscopic common bile duct exploration.Methods In 80 cases of laparoscopic common bile duct exploration,38 cases were treated with knot-free suture for primary suture of the common bile duct （observation group）,and 42 cases were treated with T-tube drainage（control group）.The intra-and postoperative conditions of the patients were compared between groups.Results Suture time,operative time and hospitalization of the control group were significantly longer than those in the observation group（P ＜0.05）.There were no significant differences in intestinal function recovery,peritoneal drainage and biliary leakage between the groups.Conclusion Application of knot-free suture for primary suture of laparoscopic common bile duct exploration can markedly reduce the suture time.It has the characteristics of simple operation,short hospitalization and low biliary leakage incidence.
