慢性梗阻性肾病进展的相关因素分析

目的:进一步探讨慢性梗阻性肾病病程进展的危险因素.方法:对337例慢性梗阻性肾病患者的临床资料进行回顾性分析.利用计算机软件包SPSS 10.0对各变量做统计分析,对各因素进行Logistic分析.结果:在影响梗阻性肾病进行性肾损害的33个因素中,经过单因素Logistic回归和多因素Logistic回归,最后性别(OR=1.963,P=0.018)、高血压(OR=2.228,P=0.039)、高尿酸血症(OR=4.204,P＜0.001)、低钙血症(OR=2.612,P=0.031)、蛋白尿(OR=1.637,P=0.049)、低蛋白血症(OR=5.055,P=0.016)、SIRS(OR=6.113,P=0.000)、贫血(OR=5.257,P=0.000)、肾萎缩(OR=5.964,P=0.009)、造影剂(OR=6.111,P=0.000)、酸中毒(OR=3.329,P=0.003)11个因素进入回归方程.结论:性别、高血压、高尿酸血症、低钙血症、蛋白尿、低蛋白血症、SIRS、贫血、肾萎缩、造影剂、酸中毒可能是慢性梗阻性肾病进行性肾损害独立的危险因素.而年龄、病程、积水程度、尿路感染、复杂性尿路梗阻度等,可能对慢性梗阻性肾病进行性肾损害有影响.

Risk Factor of Progression of Obstructive Nephropathy

Objective:To study the risk factors of the progression of chronic obstructive nephropathy.Methods:A retrospective analysis of the patients with chronic obstructive nephropathy was performed. Multivariate logistic regression was used to identify correlate risk factors of the chronic obstructive nephropathy progression. Statistic analysis was conducted with the aid of SPSS 10.0 software package.Results:In the analysis of risk factors of the chronic obstructive nephropathy progression, 11 factors including gender (OR=1.963,P=0.018), hypertension (OR=2.228,P=0.039), hyperuricemia(OR=4.204,P=0.000), hypocalcemia(OR=2.612,P=0.031),proteinuric(OR=1.639,P=0.049), hypoalbumia(OR=5.505,P=0.000),SIRS(OR=6.113,P=0.000), anemia (OR=5.257,P=0.000), renal atrophy (OR=5.964,P=0.009), contrast medium (OR=6.113,P<0.001), acidosis (OR=3.329,P=0.003) entered the equation.Conclusion:Sex, hypertension, hyperuricemia, hypocalcemia, proteinuria, hypoproteinemia, SIRS, anemia, renal atrophy, contrast medium, acidosis are the important risk factors of progression of chronic obstructive nephropathy. Age, course of disease, degree of hydrocele and urinary tract infection may increase the risk of progression of chronic obstructive nephropathy.
