高血压、冠心病患者血清同型半胱氨酸和超敏C反应蛋白水平的变化及临床意义

摘    要:目的 观察血清同型半胱氨酸(Hcy)及超敏C反应蛋白(hs-CRP)在高血压、冠心病患者中的变化,并探讨其临床意义.方法 选择我院心内科于2015年1月至2016年1月期间收治的46例高血压合并冠心病患者为研究对象,将其分为稳定型心绞痛(SAP)组26例,急性冠脉综合征(ACS)组20例,选择未合并冠心病的高血压患者25例纳入高血压组,以及25例健康体检者纳入对照组,检测并比较各组受检者的血清Hcy及hs-CRP水平.结果 血清Hcy、hs-CRP在对照组(9.83±2.12)μmol/L、(1.42±0.78)mg/L]、高血压组(14.82±3.05)μmol/L、(5.98±2.24)mg/L]、SAP组(20.32±4.08)μmol/L、(9.85±3.29)mg/L]及ACS组(23.24±4.27)μmol/L、(14.35±4.79)mg/L]均依次升高,差异均有统计学意义(P<0.05);相关性分析结果显示,血清hs-CRP与Hcy呈显著正相关(r=0.595,P<0.05).结论 高血压患者血清Hcy、hs-CRP浓度明显升高,在高血压合并冠心病患者中升高更加明显,其可反映患者病情的严重程度.

Abstract:Objective To investigate the changes and clinical significance of serum homocysteine (Hcy) and high-sensitivity C-reactive protein (hs-CRP) in hypertension and coronary heart disease. Methods Forty-six patients with hypertension and coronary artery disease treated in Department of Cardiology in our hospital from January 2015 to January 2016 were studied, and they were divided into stable angina pectoris (SAP) group (n=26) and acute coronary syndrome (ACS) group (n=20). Twenty-five hypertensive patients without coronary heart disease were enrolled as hyper-tension group, and 25 healthy subjects were included into the control group. The serum Hcy and hs-CRP levels were de-tected and compared. Results The serum Hcy, hs-CRP in the control group (9.83 ± 2.12)μmol/L, (1.42 ± 0.78) mg/L], hypertension group (14.82±3.05)μmol/L, (5.98±2.24) mg/L], SAP group (20.32±4.08)μmol/L, (9.85±3.29) mg/L] and ACS group (23.24±4.27)μmol/L, (14.35±4.79) mg/L] were successively increased, and the differences were statistical-ly significant (P<0.05). Correlation analysis revealed that serum hs-CRP and Hcy was of significant positive correlation (r=0.595, P<0.05). Conclusion The serum Hcy, hs-CRP concentrations in hypertension is significantly increased, and it is increased more obvious in hypertension with coronary heart disease, which can reflect the severity of the disease.
