脑出血早期外周血白细胞计数、分类的变化及其意义

摘    要:目的:观察脑出血患者早期外周血白细胞(WBC)计数、分类的变化,探讨其与患者预后的关系。方法:选择脑出血住院患者102例,按照出血量大小、是否破入脑室和血肿扩大分为出血量<25ml、出血量≥ 25ml、继发脑室出血和血肿扩大4组。发病24h内完成外周血WBC计数、分类的检测,并同正常对照组比较。结果:脑出血患者外周血WBC和中性粒细胞(NEUT)计数均高于正常对照组(P<0.05~P<0.01),淋巴细胞计数均低于正常对照组(P<0.05~P<0.01),继发脑室出血组的外周血WBC和NEUT计数高于出血量<25ml组(P<0.01)。结论:脑出血患者早期外周血WBC开始增高,其增高程度同出血量大小和破入脑室有关,是脑出血患者预后不良的预测指标。

Abstract:Objective:To observe the value of peripheral white blood cell (WBC) counting and classification in the early stage of intracerebral hemorrhage and its relationship with the prognosis of the patients. Methods : One hundred and two patients were classified into four groups according to the hematoma volume ( 〈 25 ml or 〉I 25 ml ), ventricular rupture and hematoma enlargement. The peripheral WBC counting and classification were performed within 24 hours from onset and the results were compared with those of the normal controls. Results:The mean peripheral WBC and neutrophil counts were significantly higher in the patients with intracerebral hemorrhage than that in the controls (P 〈 0.05 to P 〈 0.01 ) ; the mean lymphocyte count was significantly lower than the controls (P 〈 0.05 to P 〈0.01 ) ;the mean peripheral WBC and neutrophil counts were significantly higher in the ventricular rupture group than in the hematoma volume 〈25 ml group ( P 〈0.01 ). Conclusions:The peripheral WBC starts to increase in the early stage of intracerebral hemorrhage. The degree of leukocytosis is related to the size of the hematoma and ventricular rupture. It may predict poor outcome.
