艾滋病与非艾滋病患者铜绿假单胞菌感染临床及耐药对比研究

【摘要】〓目的〓探讨艾滋病与非艾滋病患者铜绿假单胞菌感染病例感染途径、临床表现及药敏结果的差异性,以期提高对相关疾病的临床诊疗水平。方法〓选取2010年1月~2014年8月32例艾滋病患者合并铜绿假单胞菌感染病例作为观察组,另选同时期27例非艾滋病患者铜绿假单胞菌感染病例作为对照组。对比分析两组患者感染途径、临床表现、耐药率的差异性。结果〓艾滋病及非艾滋病患者铜绿假单胞菌感染主要感染途径均为呼吸道。观察组临床症状以发热、咳嗽、咳痰为主,对照组临床症状相对更复杂。观察组分离出耐药菌株20株(62.5%),对照组分离耐药菌24株(88.9%)(P<0.05)。观察组菌株对亚胺培南及美洛配能较敏感,对照组菌株对阿米卡星、头孢噻肟及美洛配能敏感。结论〓艾滋病与非艾滋病患者铜绿假单胞菌感染在临床表现及耐药性方面有差异,临床上须综合分析患者病情,选择敏感抗生素。

Comparative study of clinical features and drug tolerance on pseudomonasaeruginosa in AIDS and non-AIDS patients

Objective To investigate differences in infective pathway, clinical manifestations and drug susceptibility results of pseudomonas aeruginosa infections between AIDS and non-AIDS patients, for improving diagnosis and treatment. Methods Totally 32 AIDS patients with pseudomonas aeruginosa infections from Jan.2010 to Aug.2014 were selected as the research subjects in observation group,and another 27 non-AIDS patients with pseudomonas aeruginosa infections during the same period were selected as the control group.Differences of two groups in infective pathway,clinical manifestations and drug susceptibility results were compared and analyzed.Results The primary infective pathway of two groups was the same and derived from respiratory tract infection.The main clinical manifestations of observation group were fever,cough and sputum.The clinical manifestations of control group were more complicated. Twenty strains（62.5%） of pathogens were isolated from the patients of observation group,and 24 strains（88.9%） were isolated（P〈0.05） from control group.In the observation group, P.aeruginosa had a low resistance to imipenem and meropenem.In the control group,P.aeruginosa had a relatively low resistance to amikacin,cefotaxime and meropenem.Conclusion The difference of clinical manifestation and drug susceptibility results were significant between pseudomonas aeruginosa infections between AIDS and non-AIDS patients.Clinician should analyse the different conditions of patients comprehensively and choose the antibiotics reasonably.
