高海拔地区前列腺增生患者行等离子前列腺汽化电切术与经尿道前列腺电切术的比较

目的 比较高海拔地区前列腺增生（BPH）合并高原性疾病患者行经尿道双极等离子前列腺电切术（PKRP）与经尿道前列腺电切术（TURP）治疗效果.方法 对甘孜藏族自治州人民医院2008年6月至2012年3月156例前列腺增生合并高原性疾病患者（PKRP组78例,TURP组78例）进行回顾性研究,比较两组年龄、前列腺体积、病程、前列腺特异抗原（PSA）、手术时间、术中出血量、尿管留置时间、术后膀胱冲洗时间、住院天数、术后血钠浓度、术后并发症、术前及术后国际前列腺症状评分（IPSS）、残余尿（RUV）、最大尿流率（Qmax）、生活质量评分（QOL）等.结果 两组患者年龄、前列腺体积、病程、PSA、术前及术后IPSS评分、残余尿、最大尿流率、生活质量评分、手术时间、住院天数均无统计学差异（P＞0.05）.PKRP组术中出血量、留置尿管时间、术后膀胱冲洗时间、术后并发症发生率均低于TURP组,术后血钠浓度高于TURP组（P＜0.05）.结论 对于高原BPH患者这一特殊群体,TURP、PKRP治疗疗效相当,但对于手术安全性,手术并发症的控制,PKRP术较TURP术有较多优势,对该类患者应优先选择PKRP术.

Efficacy of plasmakinetic resection of prostate compared with transurethral resection of prosstate on benign prostate hyperplasia in high altitude areas

Objective To compare the efficacy of transuretheral plasmakinetic resection of prostate （PKRP） and transurethral resection of prostate （TURP） for benign prostatic hyperplasia （BPH） patients who complicated with altitude disease in high altitude areas.Methods A total of 156 patients with BPH complicated with altitude disease were enrolled in Ganzi Tibetan Autonomous Prefecture People＇s Hospital form June 2008 to March 2012,78 cases underwent PKRP and 78 cases underwent TURP.Age,course of disease,prostate volume,PSA,operative time,intraoperative blood loss,time of postoperative foley＇s catheter retention,bladder irrigation time,days in hospital,hemo-sodium concentration of postoperation,complications of operation,pre-and post-operative international prostate symptom score,residual urine volume （RUV）,maximum flow rate （Qmax） and quality of life （QOL） were compared between groups.Results There were no significant differences between PKRP and TURP group in age,course of disease,prostate volume,PSA,pre-and post-operative IPSS,RUV,Qmax,QOL,operation time and days in hospital （P〉0.05）.Compared with the TURP group,the PKRP group had less blood loss in operation,time of postoperative foley＇s catheter retention,bladder irrigation time and lower rate of postoperative complications.Meanwhile the hemo-sodium concentration in PKRP group was higher than TURP group （P〈0.05）.Conclusion Both surgical procedures are efficient for the patients with BPH in high altitude areas.However,PKRP has more advantages.So PKRP is a better choice for those patients.
