血必净注射液辅助治疗脓毒症的临床疗效观察

目的：观察血必净注射液辅助治疗脓毒症患者的临床疗效及其不良反应。方法收集2011年2月至2013年8月本院呼吸与危重症医学科（RICU）收治的90例脓毒症患者，按照随机数字表法分为治疗组（45例）和对照组（45例）。两组患者均参照2008年美国重症医学会制定的脓毒症治疗指南进行常规治疗；治疗组在此治疗的基础上加用血必净注射液100ml静脉输注，2次/d，7 d为1个疗程。治疗前后均对所有患者进行APACHEⅡ评分，测定治疗前及治疗7 d后两组患者的降钙素原（PCT）、C-反应蛋白（CRP）水平和血常规指标；采用SPSS 17.0软件包分析两组患者的临床有效率、病死率、PCT、CRP水平、白细胞总数以及APACHEⅡ评分。结果治疗组患者治疗7 d后PCT、CRP、白细胞总数均较对照组显著下降（P 均＜0.05），治疗组APACHEⅡ评分（8.72＆#177;6.23分）较对照组（11.59＆#177;5.66分）显著下降（t =5.14，P ＜0.05）；治疗组有效率（62.2%）高于对照组（40.0%）（χ2=4.447，P＜0.05）；治疗组（17.8%）与对照组（22.2%）患者病死率差异无统计学意义（χ2=0.27，P＞0.05）。结论血必净注射液辅助治疗脓毒症的疗效确切肯定，可减轻炎症因子水平，进而提高脓毒症治疗有效率。

Clinical observasion of sepsis diseaseadjunctive treated with Xuebijing injection

ObjectiveTo evaluate theclinicaleffect of Xuebijing（XBJ） injectionintherapy of sepsis and its side effect.Method Total of 90patientswithsepsis who were diagnosed in our division ofRICU.Theywere divided intotwo groupsinrandomly. The patients incontrol group were given only common therapy, patients in therapeutic group were given with not only common therapy but also XBJ injectable. 100 ml XBJ injectable was inject mix with normal saline,twice a day,seven days.Thelevelof PCT,CRP,WBC andAPACHEⅡscoreandeffective ratewere analyzed by statistics-software SPSS17.0. ResultAfter seven days treatment.The clinical total effective rate was increased in patients of therapeutic group thanthatof in control group （χ2=4.447,P〈0.05） . The PCT,CRP,WBC level and APACHEⅡscore all decreased in two groups, and it was more decrease in therapeutic group thanthat of control group（allP＆lt;0.05）,there were no differenceinmortality betweenthe two groups （χ2=0.278,P＆gt;0.05）.Conclusions The curative effect of XBJ jection is conifrmed, which could reduce the levels of inlfammatory factors and improve.
