足底内侧神经检测在2型糖尿病周围神经病诊断中的价值分析

目的探讨足底内侧神经检测对糖尿病周围神经病的诊断价值。方法选取2020年1-7月就诊于大连市中心医院内分泌科,明确诊断2型糖尿病的109例患者为研究对象,应用肌电诱发电位仪对患者胫神经,足底内侧神经,腓肠神经,腓浅神经进行检测。结果足底内侧神经检出异常率均高于腓肠神经、腓浅神经和胫神经H反射及F波异常率,差异均有统计学意义(P<0.05)。有症状组与无症状组双侧足底内侧神经感觉神经动作电位波幅比较,差异有统计学意义(P<0.05)。结论对于糖尿病周围神经病患者,足底内侧神经检测的敏感性高于胫神经.腓肠神经和腓浅神经,建议在常规神经传导检测基础上进行足底内侧神经检测,以有利于糖尿病周围神经病的早期诊断,这对于患者治疗和预后具有重要意义。

The Value of Medial Plantar Nerve Detection in the Diagnosis of Type 2 Diabetic Peripheral Neuropathy

Objective To discuss the diagnostic value of medial plantar nerve detection in diabetes peripheral neuropathy(DPN).Methods A total of 109 patients with type 2 diabetes mellitus who were admitted to the department of endocrinology of Dalian Municipal Central Hospital from January to July 2020 were selected as the research objects.The tibial nerve,medial plantar nerve,sural nerve and superficial peroneal nerve were detected by EMG.Results The abnormal rate of medial plantar nerve was significantly higher than those of sural nerve,superficial peroneal nerve and tibial nerve in H reflex and F wave(P<0.05).There was significant difference between symptomatic group and non symptomatic group bilateral medial plantar nerve sensory nerve action potential amplitude(P<0.05).Conclusion For patients with diabetic peripheral neuropathy,the sensitivity of medial plantar nerve detection is higher than that of tibial nerve,sural nerve and superficial peroneal nerve.It is suggested that the detection of medial plantar nerve on the basis of conventional nerve conduction test is conducive to the early diagnosis of diabetic peripheral neuropathy,which is of great significance for the treatment and prognosis of patients.
