自制脂膜氟烷超声造影剂对实验性肾肿瘤显影效果的研究

摘    要:目的 探讨自制脂膜氟烷超声造影剂“脂氟显”对兔肾VX2 肿瘤的显影效果。方法 对 16只肾肿瘤模型经兔耳缘静脉团注“脂氟显”0 .0 2ml/kg进行超声造影 ,并与增强CT、病理对照研究。结果  16只兔肾病理检查共有5 1个肿瘤 ,超声造影后检出 46个 ,检出率 90 .1% ,增强螺旋CT检出 45个 ,检出率 88.2 %。超声造影和增强CT对模型兔肾肿瘤的检出率无统计学差异 (P >0 .0 5 ) ,两者漏诊的肿瘤直径均小于 0 .3 2cm。造影前超声假阳性 5个 ,假阳性率13 .8% ,CT平扫无假阳性。结论 超声造影剂“脂氟显”能有效增强兔肾肿瘤的显像 ,提高肾脏肿瘤的检出率和特异性 ,其诊断价值与增强螺旋CT相当

Abstract:Objective To investigate the effect of contrast-enhanced second harmonic imaging(SHI) in detection of renal VX 2 tumor using a self-made liposome perfluocarbon ultrasound contrast agent named "ZhiFuXian". Methods Sixteen rabbits with VX 2 tumor were scanned by fundamental imaging and SHI before and after intravenously bolus injection "ZhiFuXian" in a dose of 0.02ml/kg ,and followed by helical CT scan(HCT),including general HCT and enhanced HCT, and pathology examination. Results In 51 gross lesions demonstrated by pathology examination, contrast-enhanced SHI detected 46 lesions and enhanced HCT 45,the detection rates were 90.1%,88.2%,respectively. There was no statistically significant difference (P>0.05). The tumor diameters undetected both by contrast-enhanced SHI and enhanced HCT were less than 0.32cm. There were 5 false positive lesions in ultrasonography without contrast agent , but no false positive lesions in general HCT. Conclusion The ultrasound contrast agent "ZhiFuXian" can effectively enhance the imaging of rabbit renal tumors and improve the detection rate and specificity, and be equal to enhanced HCT in diagnostic value.
