经颅多普勒超声检测对脑出血患者预后的预测价值

摘    要:目的探讨经颅多普勒超声(TCD)检测对脑出血患者病情程度及预后的预测价值。方法选择首次发病且发病24h内入院的54例脑出血患者,分别于入院第1、7、21d进行床旁TCD检测,并分别对TCD的主要指标平均血流速度(Vm)及脉动指数(PI)与脑出血体积、神经功能缺损评分(MESSS)、日常生活能力评分(BI)进行相关分析。结果TCD异常主要表现为高阻力、低流速的血流频谱。连续监测发现,降低的血流速度逐渐增高,同时增高的脉动指数则逐渐降低。出血量、MESSS评分与大脑中动脉的Vm呈负相关,与PI呈正相关;而BI与大脑中动脉的Vm呈正相关,与PI呈负相关。结论TCD指标异常程度有助于判断脑出血患者的病情及评估预后。

Abstract:Objective To investigate the prognostic value of TCD detection in intracerebral hemorrhage (ICH) pa- tients. Methods We enroued 54 patients with a first acute ICH with 24h after onset.All the patients were dynamic evaluated by beside TCD on 1st,7th,21st day.And the celation between the primary parameter of TCD,Vm and PI,and bleeding volume, MESSS,Barthel index (BI) were studied.Results The TCD abnormity was high-resistance,low-flow velocity blood wave- forms.Continuous monitoring revealed that,the low velocity of the cerebral blood flow increased,meanwhile the high value of PI decreased gradually.The volume of bleeding,MESSS scale had negative correlation with the Vm the MCA,but positive correla- tion with the PI value.While the BI scale was positive correlation with the Vm of MCA,negative correlation with the PI value. Conclusion The degree of TCD parametric abnormity win redound to the judgement of state of and assessment of it prognosis.
