输血次数与交叉配血次侧凝集关系研究

目的探讨输血次数与微柱凝胶配血次侧凝集间的关系。方法回顾性分析自2015年1月至2016年9月第二军医大学附属二院长征医院收治的318例多次输血患者的临床资料,分析累计1 250次微柱凝胶配血检验记录。结果 318例患者中,第一次输血时呈现次侧凝集阳性率为48.7%(155/318),此后检测过程中出现过次侧凝集阳性率为60.4%(192/318)。后续输血过程中有60例初始阴性患者发生此次凝集,其中,51例是在第2、3次输血时发生凝集;另有23例首次检验阳性患者后续转为阴性。结论重复输血可引发交叉配血次侧凝集,但并不是充分条件,对重复输血患者需要在初期备血阶段就加强对次侧凝集检测的关注度。

Relationship between number of blood transfusions and agglutination in secondary-side of cross matching

Objective To investigate relationship between number of blood transfusions and agglutination in secondary-side of cross matching.Methods A retrospective study was performed on 1 250 gel microcolumn gel tests of 318 patients who received multiple transfusion from January 2015 to September 2016.Results In 318 cases of patients,blood transfusion for the first time presenting the primary side agglutination positive rate was 48.7％ (155/318),then 60.4％ (192/318).Subsequent blood transfusion in the process of 60 primary negative patients,51 of whom made the change during the second or third transfusion,showed positive results later on,while 23 primary positive patients convert to negative ones.Conclusion Repeated transfusion may lead to agglutination in secondaryside of cross matching,not a sufficient condition.Special attention should be paid to relate tests since the very beginning of blood-preparation for patients requiring multiple transfusions.
