全膝关节置换术后应用局部间断冰敷治疗效果分析

目的探讨全膝关节置换术后应用局部间断冰敷治疗的临床效果。方法选取2012年11月至2016年11月行全膝关节置换术的患者51例(51膝),其中,术后应用局部间断冰敷治疗(冰敷组)29例,常规治疗(常规组)22例。观察并比较两组患者术后第1、2、3天的膝关节术前术后髌骨中点周径差、视觉模拟疼痛评分(VAS)及总引流量。结果术后第2、3天冰敷组膝关节术前术后髌骨中点周径差明显小于常规组(P<0.05),术后第1、2、3天冰敷组VAS明显低于常规组(P<0.05),术后冰敷组总引流量明显少于常规组(P<0.05)。结论全膝关节置换术后应用局部间断冰敷治疗可明显促进患肢局部消肿,缓解术区疼痛,减少术后出血。

Application of local discontinuous ice compress after total knee arthroplasty

Objective To explore the effect of application of local discontinuous ice compress after total knee arthroplasty (TKA).Methods A retrospective study was performed on 51 patients who underwent TKA from November 2012 to November 2016.Patients were divided into the local discontinuous ice compress group(ice compress group,n =29),and the conventional therapy group(observation group,n =22).The circumference and diameter of patellar midpoint,visual analog pain score(VAS) and total volume of drainage were observed and compared between the two groups on 1,2 and 3 days after TKA.Results The circumference and diameter of patellar midpoint was obviously shorter than that in the observation group on postoperative 2 and 3 days (P ＜ 0.05),VAS score in the ice compress group was less than that in the observation group on postoperative 1,2 and 3 days (P ＜ 0.05),and the total volume of drainage in the ice compress group was obviously less than that in the observation group (P ＜ 0.05).Conclusion The local discontinuous ice compress treatment after total knee arthroplasty can obviously promote the limb local swelling,significantly relieve postoperative pain and reduce blood loss.
