不同心功能情况对经皮冠状动脉介入治疗术后患者1年出血事件影响

目的探讨不同心功能情况对经皮冠状动脉介入治疗(PCI)术后1年出血事件发生情况的影响。方法回顾性分析2007—2014年在沈阳军区总医院住院治疗并行PCI术的纽约心脏病协会(NYHA)心功能分级Ⅰ~Ⅲ级及心功能正常的冠心病患者的临床资料。所有入选患者经过倾向性评分匹配后,最终筛选出4 145对(8 290例)患者。根据NYHA心功能分级标准将所有患者分为症状组(NYHA分级Ⅱ~Ⅲ级,n=4 145)及对照组(心功能正常及NYHA分级Ⅰ级患者,n=4 145)。所有患者均行PCI治疗,随访1年,观察并比较两组患者出院1年出血事件的发生情况。结果两组患者一般资料、造影结果及介入手术资料、全部出血事件比较,差异均无统计学差异(P>0.05);两组患者出血学术研究联合会(BARC)2~5级、BARC 3~5级出血事件发生率比较,差异也无统计学差异(P>0.05)。结论单中心资料表明,心功能严重程度不影响PCI术后患者出血事件的发生。

The effect of different cardiac function in patients with percutaneous coronary intervention after operation on the bleeding events in the last 1 year after discharge

Objective To investigate the effect of different cardiac function on the occurrence of bleeding after percutaneous coronary intervention(PCI) for 1 year.Methods A retrospective study was performed on patients with coronary heart disease who were admitted from 2007 to 2014 in the treatment of PCI,and the conditions of the entry group were according to heart function(NYHA class Ⅰ-Ⅲ).All 8 290 patients were divided into the symptom group(NYHA class Ⅱ-Ⅲ,n=4 145) and the control group(including normal heart function and NYHA I,n=4 145).All patients were discharged after percutaneous coronary intervention(PCI).Patients were followed up for 1 years.The incidence of bleeding events in the two groups were observed and compared.Results There was no significant difference in baseline data,angiographic results,interventional procedures and total bleeding events between the two groups(P>0.05).Comparison of BARC 2-5 bleeding events and the 3-5 level had no statistically significant difference(P>0.05).Conclusion The single-central data showed that the severity of cardiac function did not affect the occurrence of bleeding events after PCI.
