截瘫患者排尿及阴茎勃起功能恢复的临床研究

目的 探讨恢复或改善脊髓损伤性截瘫患者排尿功能及阴茎勃起功能的方法。方法 倒脊髓损伤性截瘫患者施行大网膜脊髓圆锥部移植手术前后，进行尿流动力学和夜间电子生物阻抗测量装置（NEVA）检测。结果 5例患者其最大尿流率平均由术前3．3ml／s到术后16．4ml／s，膀胱残余尿量平均由术前204．1ml到术后26．7ml；NEVA系统检测夜间阴茎勃起次数平均由术前1．6次到术后3．5次，阴茎勃起的最大体积改变由术前平均168．4％到术后平均256．6％。结论 对脊髓损伤性截瘫患者，利用未损伤的圆锥施行大网膜移植术，激发其排尿功能及阴茎勃起功能的恢复，是大网膜治疗脊髓损伤又一新的开拓和创新，是一个新的重要理念。

A Clinical Study of Restoring Urination and.Erectile Function in Patients with Paraplegia

Objective To study the methods of restoring and improving urination and erectile function in patients with traumatic paraplegia.Methods Before and after being transplanted omentum into cone of spinal cord,five patients with paraplegia were respectively examined by urodynamics and nocturnal electro-bioimpedence volumetric assessment (NEVA) and other specific examinations.Results Micturition function and penile erectile function of the patients were improved by transplanting of omentum into cone of spinal cord,and maximum flow rate changed from 3.3 ml/s before operation to 16.4ml/s after operation,residual urine volume from 204.1 ml before operation to 26.7ml after operation.And the number of erectile events increased from 1.6 before operation to 3.5 after operation and the percentage of volume change over baseline also increased from 168.4% to 256.6%.Conclusions It is a new and important idea to stimulate cone of spinal cord to improve urination and erectile function by transplantation of omentum into no traumatic cone of spinal cord for patients with traumatic paraplegia.
