23例气管、支气管、隆突成形术的体会

目的 报告23例气管、支气管、隆突成形术。方法 右肺上叶袖式切除13例。左肺上叶袖式切除4例(同时行肺动脉楔形切除1例)，右全肺袖式切除 隆突成形3例，隆突切除、呼吸道重建1例，左总支气管部分切除、端端吻合1例，气管肿瘤切除、端端吻合术1例。结果 全组术后近期死亡1例，手术死亡率4．3％，死亡原因为呼吸衰竭。结论 气管、支气管、隆突成形术安全，易于掌握，能最大限度切除病变组织且最大限度保留有功能的肺组织，符合肿瘤治疗原则。

The experience of the plasty of trachea,bronchus and carina in 23 patients

Objective To report 23 cases of the clinical results of the plasty of trachea,bronthus and carina.Methods The operation style of the group includes 13 cases of sleeve resection of the right upper lobe,4 cases of sleeve resection of the left upper lobe(including 1 case with pulmonary artery wedge excision),3 cases of right sleeve pneumonectony+carina plasty,1 case of resection of trachea carinoma and carina+air passage reconstructing,and 1 case of segment resection of left main stem bronchus(including the foreign bodies)+end to end anastomosis of left stem bronchus.Results Because of respiration failure,one case is death recently after operation.The mortality rate of the operation is 4 3% Conclusion The plasty of trachea,bronchus and carina is safe and easy to control,and can resect the tissue of pathological changes to maximum and at the same time retain the functional lung tissue to maximum.
