超声对上消化道穿孔的应用价值

探讨超声对上消化道穿孔的应用价值。方法：收集60例以上消化道穿孔为出院诊断的患者病历资料，包括首诊超声、立位腹平片及手术结果或保守治疗后复查的超声资料，探讨超声对上消化道穿孔的应用价值。结果：60例患者超声诊断正确56例（十二指肠球前壁穿孔30例，胃窦部穿孔26例），4例漏诊（2例为球后壁穿孔，2例为胃窦部穿孔）；25例同时发现了清晰的穿孔孔道、游离气体、腹腔积液，20例同时看到了直接的气液外溢现象；4例仅发现了肝下缘少量积液；45例直接进行了手术治疗，11例保守治疗有效，4例保守治疗无效后手术治疗。结论：超声不仅可以对上消化道穿孔作出准确的诊断，而且可以指导临床做出进一步治疗方案。

Value of Ultrasound Examination in Upper Gastrointestinal Perforation

Objective ：To discuss ultrasonic application value on upper gastrointestinal perforation .Methods ：Analyze statistics of 60 patients about data of first diagnostic ultrasound ,stand abdominal plain film and surgery results or ultra-sonic information after conservative treatment ,who saw a emergent doctor because of the sudden abdominal discomfort with board-like rigidity of the abdomen between September 2009 to June 2012 ,and summarize the value of ultrasonic on how to diagnose gastrointestinal perforation .Results：56 cases from 60 cases we have done made a correct diagnosis . Conclusion：Ultrasound not only can make a correct diagnosis on upper gastrointestinal perforation but also can provide information on definite parts and size of perforation ,and make a diagnosis that perforation has closed or not according to ultrasonic review data of actual time ,so conduct the clinical to make a further therapeutic schedule .
