IOLMaster测量下SRKⅡ和第三代人工晶体计算公式的比较

摘    要:目的以IOLMaster(光学相干生物测量仪)行眼部参数测量,通过分析研究不同眼轴长度患者,比较SRKⅡ公式和第三代人工晶体计算公式的精度。方法年龄相关性白内障患者121例135只眼,术前应用IOLMaster测量眼部参数,分别代入SRKⅡ公式和第三代SRK—T、Haigis、Holladay、HofferQ公式计算人工晶体度数。并将所有眼分为短眼轴组(L≤22mm),正常眼轴组(22mm〈L≤26mm)和长眼轴组(L〉26mm)。以术后3月所测患者的实际屈光度来比较不同眼轴长度下各公式的术后绝对屈光误差值(ME)。结果长眼轴组共44眼,SRKⅡ公式与Haigis、SRK—T公式间有显著性差异(P〈0.05或P〈0.01),而与Holladay、HofferQ公式间无统计学差异。在正常眼轴组和短眼轴组,各公式间无显著性差异。结论在正常眼轴患者,SRKⅡ公式和第三代公式表现出同样较高的精度,但在长眼轴组,SRKⅡ公式的精度显著低于Haigis和SRK—T公式。故建议,对于长眼轴白内障患者选用第三代人工晶体计算公式。

Abstract:Objective To compare the accuracy of SRK Ⅱ formula and the third theoretical formula by analysing patients with different axis oeuli detected by intraocular lens (IOL) Master. Methods The parameters of eyes of 121 patients ( 135 eyes) with age-related cataracts were measured by IOLMaster preoperatively. IOL power was calculated according to SRK-Ⅱ , SRK-T, Haigis, Holladay and HofferQ formula respectively. All patients were divided into three groups, the short ocular axis group( L≤22 mm), the normal axis group(22 mm 〈 L≤26 mm) and the long axis group( L 〉 26 ram). The mean absolute refractive error of patients with different axis were compared according to the accurate refraction 3 months after operation. Results There were significant differences between SRK-Ⅱ and SRK-T, Haigis formula, and there was no difference between SRK-Ⅱ and Holladay, HofferQ formula in long axis group. There was no significant difference among all the formulas in normal axis and short axis group. Conclusions SRK-Ⅱ formula is as accurate as the third theoretical formula for patients with normal axis. However, the SRK-Ⅱ formula performed with less accuracy than SRK-T, Haigis formula in patients with long axis. It suggests that patients with long axis should use the third theoretical formula.
