机械通气并发院内感染的危险因素分析

目的　探讨重症监护病房机械通气并发院内感染的危险因素。方法　按有无院内感染 ,将 70例机械通气患者分成感染组和非感染组 ,对照分析肺部感染与机械通气天数、气管插管及切开、留置胃管、H2 受体阻滞剂的使用和基础疾病的关系 ,并分析菌种。结果　 2 0例发生了院内感染 ,其中 14例为肺部感染 (占 2 8% )。感染组与非感染组在上述各危险因素方面相比有显著差异。菌种以假单孢菌为主 (占 37.5 % )。结论　院内感染与机械通气、气管插管、留置胃管、H2 受体阻滞剂的使用和基础疾病密切相关

Analysis of Risk Factors of Ventilator-associated Hospital Acquired Infections

Objective To evaluate the risk factors of ventilator-associated hospital acquired infections in Intensive Care Unit (ICU).Methods 70 cases who received mechanical ventilation were divided into two groups:infection and noninfection,according to the presence of hospital acquired pulmonary infections.Comparative study on the relationship of pulmonary infection with the time duration of mechanial ventilation,basic diseases,tracheal intubation,dwelling stomach tube and the use of H 2-receptor blocking agents was carried out and at the same time,the kind of bacteria was also analysed.Results 20 out of 70 patients developed nosocomial infections with an infection rate of 28%,and 14 of them developed lower respiraory infection.The difference between infection group and noninfection about the above-mentioned risk factiors was significant.Among the bacteria,pseudomonas took the first place,its proportion rate being 37.5%.Conclusion Pulmonary nosocomial infections have significant relationship with the following risk factors:the time duration of mechanical ventilation,basic diseases,tracheal intubation,dwelling stomach tube and the use of H2-receptor blocking agents.
