摘    要:目的比较基质金属蛋白酶-13(MMP-13)在退变和创伤椎间盘髓核中的不同表达,探讨其在椎间盘退变过程中的作用。方法随机选择2011年1—7月在宁波大学附属医院因腰椎间盘突出症行髓核摘除的33例患者(退变组)和因外伤性脊椎骨折行内固定的6例患者(对照组)作为研究对象。采用免疫组织化学方法检测患者髓核组织的MMP-13表达,应用Image-ProPlus5.1图像分析系统测定其灰度值。结果对照组椎间盘组织中MMP-13的表达很少,平均阳性表达率为(20.4±14.3)%。退变组椎间盘组织中MMP-13的表达呈弥漫性或局灶性强染色,平均阳性表达率为(73.2±10.9)%。退变组的MMP-13阳性表达率显著高于对照组(P<0.01)。结论椎间盘退变髓核组织中MMP-13的表达显著升高,MMP-13的异常表达可能在椎间盘退变发病过程中发挥着重要的作用。

Abstract:Objective To compare the expression of matrix metalloproteinase 13(MMP-13) in the nucleus pulposus of degenerative and injured intervertebral disc,and to investigate the possible role of MMP-13 in the pathogenesis of intervertebral disc degeneration.Methods From January to July 2011,the nucleus pulposus was removed in 33 patients with protrusion of irtervertebral disc(degeneration group) and internal fixation was implanted in 6 patients with lumbar vertebral fracture(control group).Immunohistochemistry assay was used to detect the expression of MMP-13 in nucleus pulposus.The gray scale was measured with Image-ProPlus5.1 image analysis system.Results The expression of MMP-13 in degeneration group was significantly higher than that in control group(73.2±10.9]% vs.20.4±14.3]%,P<0.01).MMP-13 was darkly stained in degeneration group.Conclusion The expression of MMP-13 is significantly increased in the nucleus pulposus of degenerative intervertebral disc.And MMP-13 may play an important role in the development of the lumbar intervertebral degeneration.
