硝苯地平缓释片与厄贝沙坦片及哌唑嗪三联治疗难治性高血压的疗效观察

目的 探讨硝苯地平缓释片与厄贝沙坦及哌唑嗪三联疗法对难治性高血压患者的临床疗效.方法 选择2010年7月～2013年7月104例难治性高血压患者作为研究对象,根据数字法将患者随机分成观察组与对照组各52例,对照组患者服用硝苯地平缓释片及厄贝沙坦片治疗,观察组在此基础上增用哌唑嗪治疗.治疗4周后对比两组降压效果,比较两组治疗前后心率（HR）、收缩压（SBP）及舒张压（DBP）水平,以及两组治疗前后尿素氮（BUN）、肌酐（CREA）及尿酸（UA）等实验室指标.结果 观察组降压总有效率为96.15％,显著高于对照组的82.690％（P＜0.05）;两组治疗后的HR、SBP及DBP水平均下降,但观察组下降水平均显著低于对照组（均P＜0.05）;两组治疗后BUN、CREA及UA水平均有下降,但观察组下降幅度高于对照组（均P＜0.05）.结论 使用硝苯地平缓释片与厄贝沙坦及哌唑嗪三联疗法治疗难治性高血压,能有效帮助患者控制血压及心率,安全性好,对肝肾功能无明显影响,可在临床推广应用.

Curative effect of triple therapy of nifedipine sustained release tablets irbesartan tablets and prazosin for refractory hypertension patients

Objective To study curative effect of triple therapy of nifedipine sustained release tablets, irbesartan tablets and prazosin for refractory hypertension patients. Methods According to the method, 104 patients with hyper tension were randomly divided into observation group （52 cases） and control group （52 cases）. The control group was treated with nifedipine sustained release tablets and irhesartan tablets. The ohservation group treated with prazosin on the basis of nifedipine sustained release tablets and irbesartan tablets. T he heart rate, systolic blood pressure, diastolic blood pressure level, urea nitrogen, creatinine and uric acid were observed. Results The observation group＇s total effective rate （96.15%） was significantly higher than the control group （82.69% ）. The HR, SBP and DBP decreased after treatment. HR, SBP and DBP of the observation group after treatment were significantly lower than that of the control group after treatment. The BUN and CREA and UA of the observation group drop were higher than that of the control group （P〈0.05）. Conclusion Nifedipine sustained release tablets and irbesartan tablets and prazosin can effectively help patients with hypertension control their heart rate and blood pressure.
