沿乳晕切口皮瓣法分离治疗乳腺良性肿瘤的临床疗效观察

目的：观察沿乳晕切口皮瓣法分离治疗乳腺良性肿瘤的临床效果。方法300例女性乳腺良性肿瘤患者随机分为观察组与对照组，分别采用沿乳晕切口皮瓣法分离行乳腺肿块切除手术和放射状切口行乳腺肿块切除术。观察患者术后复发、切口感染、乳头塌陷、乳头坏死、皮肤拉伤或烫伤等客观指标以及总体外观满意与否、瘢痕可接受与否、乳头乳晕感觉异常有无等主观指标。结果在客观疗效方面，两组皮肤拉伤或烫伤方面比较，差异有统计学意义（χ2=7.48，P＜0.05）。在主观疗效方面，两组乳头乳晕感觉异常方面比较，差异无统计学意义（χ2=0.10， P＞0.05），在总体外观满意与否、瘢痕可接受与否方面比较，差异均有统计学意义（χ2分别=9.02、6.10，P均＜0.05）。结论沿乳晕切口皮瓣法分离治疗乳腺良性肿瘤虽可导致一定的皮肤拉伤烫伤，但在总体外观满意与否、瘢痕可接受与否方面优于传统术式，可以有效减轻术后瘢痕，改善术后乳房外观。

Clinical effect observation of flap separation with periareolar incision in benign breast tumors

Objective To observe clinical effect observation of flap separation with periareolar incision in benign breast tumors. Methods A total of 300 female patients with benign breast tumors were randomly divided into the observation group and control group. The observation group was given breast surgery through flap separation with periareolar incision while the control group was given surgical procedures through radiate incision. The objective and subjective clinical indexes such as recurrence, incision infection, nipple collapse, nipple necrosis, skin injury or burn, overall appearance satisfac-tion, scar acceptation and nipple areola paresthesia were compared. Results There was significant difference of skin in-jury or scald between two groups （χ2=7.48,P〈0.05）. No significant difference was shown in index of nipple areola pares-thesia （χ2=0.10,P〉0.05）, except for the indexes of general appearance satisfaction and scar acceptation （χ2=9.02,6.10, P〈0.05）. Conclusion Although flap separation with periareolar incision in benign breast tumors is high risk for skin in-jury or scald, it is better than traditional radiate incision in general appearance satisfaction and scar acceptation which can reduce postoperative scar and improve postoperative breast appearance.
