急性胆源性胰腺炎的外科治疗

目的 :探讨急性胆源性胰腺炎的手术时机和手术方式。方法 :回顾性分析 189例急性胆源性胰腺炎 ,根据胆道梗阻情况和胰腺炎严重程度分别采取不同的手术方案。结果 :无胆道梗阻的胰腺炎采取非手术治疗 ,胰腺炎恢复后行胆囊切除术 ,效果良好 ;合并胆道梗阻的胰腺炎早期手术 ,以解除胆道梗阻为主 ,效果良好 ;重症尤其合并如腹腔间室综合征等情况时疗效不好。结论 :外科治疗急性胆源性胰腺炎应根据胆道有无梗阻 ,结合胰腺炎严重程度制定相应的手术方式和选择恰当的手术时机。

Surgical Treatment of Acute Biliary Pancreatitis

Aim To investigate the indication and timing for operation in acute biliary pancreatitis.Methods The clinical date of 189 patientswith acute biliary pancreatitis treated in our hospital were analyzed.Results Nonoperative therapy should be adopted in non-biliary pancreatitis. Pancreatitis with biliary obstruction should be treated by early surgery. It was difficult to treat severe acute biliary pancreatitis with abdominal compartment syndrome by surgical procedures. Conclusion The choice of indication and timing for operation in acute biliary pancreatitis should be made on whther biliary obstruction or type of pancreatitis.
