平板运动试验检出无症状心肌缺血的临床意义

摘    要:目的 探讨无症状心肌缺血(SMI)患者,可能发生心脏意外的潜在危险性.方法 随机选择25例男性患者作平板运动试验,其中14例为阳性,11例为阴性.对上述患者进行3年跟踪随访,并作同等功量运动试验,进行前后对照.结果 25例患者3年跟踪随访中,原14例阳性患者经随访和运动试验复查,其中4例缺血性ST段水平压低加重,并表现劳力性心绞痛;3例反复发作心房扑动和左心衰竭的临床症状;1例已患有心肌梗死;1例随访发生心脏猝死.另5例对比运动试验无异常改变.其心脏意外发生率为64.29%.11例运动试验阴性对照组随访中无1例发生心脏意外.结论 运动诱发无症状心肌缺血(SMI)在近几年内有发生心脏意外的潜在危险性,临床上应重视防治.

Abstract:Objective To explore the potential hazard of cardiac accident in patients with silent ischemia(SMI).Methods 25 male patients were randomly selected to do plate movement test.14 patients were positive and 11 patients were negative. Those patients were done 3 years follow up and coordination quantity movement tests were also determined.Results In the 14 positive patients who tracked follow up and plate movement test rechecked. 4 patients were positive who were discovered positive ST level depression aggravate,and angina was appeared. 3 patients were recurrent attacks atrial flutter and left heart failure, 1 patient with myocardial infarction, 1 patient died due to heart failure, another 5 patients with normal transform. The rate of cardiac accidence was 64.29%. None occurred cardiac accident in 11 patients.Conclusion SMI evocated by movement test is potential hazard in cardiac accident.It shall be concidered for prevention in the clinic.
