负压封闭引流在下肢自体植皮术中应用的临床研究

目的 探讨负压封闭引流(VSD)在下肢自体植皮术中临床应用效果.方法 收集本院2012年5月至2015年4月收治的68例需行下肢自体植皮术患者,行前瞻性研究且随机分为两组,每组34例,对照组患者给予常规换药后植皮,实验组则给予负压封闭引流植皮,比较两组患者相关临床指标、术后并发症与感染类型情况.结果 实验组患者首次植皮成功率为(88.73±3.65)％,显著性高于对照组的(78.48±3.28)％,创面修复时间为(7.12±1.07)d、住院时间为(16.17±2.06)d,术后白细胞水平为(8.25±1.34)×109/L,显著性低于对照组的(10.28±1.44)d、(20.83±2.62)d、(12.74± 1.81)×109/L],差异具有显著统计学意义(P＜0.01);实验组患者植皮边缘坏死发生率(5.88％)明显低于对照组(23.43％),差异具有显著统计学意义(P＜ 0.01);实验组患者铜绿假单胞菌感染率(5.88％)明显低于对照组(26.47％),差异具有统计学意义(P＜0.05).结论 负压封闭引流在下肢自体植皮术中临床应用效果显著,且安全性较高,具有借鉴性.

Vacuum sealing drainage in lower limb autologous skin-grafting surgery

Objective To explore the effect of vacuum sealing drainage (VSD) in lower limb autologous skin-grafting surgery.Methods In the prospective study,68 patients who were going to have lower limb autologous skin-grafting surgery were randomly divided into a VSD group (n=34) and a control group (n=34).The control group was given skin graft after routine dressing change;the experimental group was given vacuum sealing drainage grafting.The related clinical indexes,postoperative complications,and infection types were compared.Results The initial graft survival rate was significantly higher in the VSD group than in the control group (88.73±3.65)％ vs.(78.48±3.28)％].The wound repairing time,hospital stays,and White blood cell levels after the surgery were significantly lower in the VSD group than in the control group (7.12±1.07) d vs.(10.28±1.44) d,(16.17±2.06) d vs.(20.83±2.62) d,and (8.25±1.34) ×109/L vs.(12.74±1.81) ×109/L],with statistical differences (P＜0.01).The incidence the necrosis of the edge of grafted skin was obviously lower in the VSD group than the control group (5.88％ vs.23.43％),with a statistical difference (P＜0.01).The infection rate of pseudomonas aeruginosa was significantly lower in the VSD group than in the control group (5.88％ vs.26.47％),with a statistical difference (P＜0.05).Conclusions The effect of vacuum sealing drainage in lower limb autologous skin-grafting surgery is remarkable and its safety is higher,which is to say,this is of reference.
